If you would like to register at Grimston Medical Centre, please call in to the Medical Centre Reception and you will be given a Registration Pack to complete. If you have your NHS Medical Card please bring this with you.
Alternatively you can download and print the Registration Form and complete as fully as possible. Once completed please return the form to Reception.
We offer a special “New Patient Health Check” to all newly registered patients: please ask at Receptionist when you register.
It may be possible for you to express a preference for a particular practitioner; however this will depend on the practitioner’s availability and list size.
